As Transitional Case Manager, Justice, you’ll work with clients to address the underlying issues that led to contact with the justice system, to provide counselling that builds on their strengths, and helps them make steps towards recovery with an aim to improve their quality of life, reduce harm, and prevent future interactions with the justice system. You’ll also be responsible for attending court and communicating with justice partners regarding progress and to advocate for the best interests of your clients.

 WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated):
    • Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)
    • For internal applicants, grid level placement will occur as per the Collective Agreement 
  • When you'll work - 70 hours over two-week period; 8:30 am to 4:30 pm, Monday to Friday, with one unpaid hour for lunch. These hours may vary on occasion. 
  • What your benefits will be - After a 90-day waiting period, will be enrolled in the extended healthcare benefits plan
  • Your pension plan - Immediately enrolled in the Healthcare of Ontario Pension Plan (HOOPP)


HOW YOU WILL BE MAKING A DIFFERENCE:

  • Perform intake at probation meetings, parole meetings, or on site at jails in Southwestern Ontario
  • Attend court sessions, as needed, and provide court support to individuals dealing with current or past charges
  • Liaise with legal professionals e.g. lawyers, probation officers, social workers
  • Liaise with court system, detention centres, community-based agencies in order to provide support and/or discharge planning upon individual’s release from custody
  • Demonstrated ability to work with individuals living with multiple challenges related to involvement with the criminal justice system, mental illness, substance use, complex health challenges, street-level sex work, poverty, homelessness, abuse and trauma
Assessment--In collaboration with the client:
  • Assist with short term crisis management by providing an urgent/emergent response to individuals in need of case management supports
  • Engage with clients on a short-term basis to support their immediate needs
  • In collaboration with the client, conduct a variety of assessments (skills, functional ability, needs, barriers, etc.)
  • Assess the adequacy of resources/supports currently available to the client
  • Determine the impact of psychiatric symptoms on functioning
  • Identify risk factors related to client and staff safety as well as relapse
  • Assess barriers to successful community living (e.g. treatment options, transportation, finances, etc.)
  • Obtain information, as needed, from other relevant sources
  • Ensure the assessment incorporates cultural and ethnic factors
  • Identify areas requiring additional assessment
Individualized Goal Setting and Rehabilitation Planning -- Utilizing the information obtained through the assessment process and in collaboration with the client:
  • Assist the client to identify their personal goals and preferences and utilize standardized tool (GAIN, OCAN, etc.)
  • Design rehab plans based on client needs, goals and preferences
  • Assist the client to navigate systems and connect to appropriate resources
  • Assist the client to explore options and consequences of different choices
  • Build on the client’s personal strengths, abilities and resources
  • Help clients work towards their vision of a positive personal future
  • Help clients identify practical strategies for achieving their vision
  • Ensure issues related to transportation are addressed, incorporated into the rehab plan and are consistent with organizational policies and practices
  • Work in partnership with the client in all aspects of planning
  • Identify intervention strategies to meet goals
  • Establish an intervention plan which includes expectations and responsibilities of the client, Case Manager, other supports (e.g. family) and resources, as well as the frequency and nature of interventions
  • Obtain necessary consents
  • Document and communicate the plan to all relevant stakeholders
Implementation of Rehabilitation Plan -- Based on identified goals and intervention strategies:
  • Provide individual skill teaching and/or support the client to obtain the necessary skills training from other sources
  • Provide needed information and education about mental illness, coping skills, services, resources, self-help and consumer initiatives/supports
  • Utilize basic communication and intervention skills, supported by empirical validity, practice guidelines and relevant standards of care for different interventions
  • Encourage and facilitate personal growth and development towards rehabilitation and recovery
  • Motivate the client to learn new skills and behaviours
  • Build on client’s personal strengths and adjust interventions based on individual response
  • Support the client to access needed resources
  • Assist the client to identify and connect with personal/natural supports
  • Meet with the client regularly to work on specific goals until other permanent supports are in place
Service Coordination and Advocacy:
  • Have knowledge about community resources relevant to clients, including other mental health services, entitlement and benefit programs, (e.g. ODSP, Ontario Works, London Housing, etc.)
  • Identify, develop and maintain good links with a wide range of community resources
  • Assist the client to obtain needed resources by educating and supporting the client to access resources, linking the client to appropriate services, advocating with or for the client when needed
  • Demonstrate knowledge of community resources and its place within the mental health care system
  • Assist in building positive working relationships across the service system
  • Participate in public education and advocacy


WHAT YOU'LL NEED TO APPLY:

We require the following qualifications:

  • Diploma or degree in a health, social service or a related discipline (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript, for ongoing education, an unofficial transcript will be accepted)
  • Valid driver's licence and access to a vehicle with adequate insurance to carry out job duties
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:

  • Current registration in good standing with a relevant professional regulatory body
  • Demonstrated knowledge and skill in clinical assessment, functional assessment, service planning, relapse prevention, and client advocacy
  • Broad knowledge of, and practice in, evidence-based treatment approaches to working with individuals with severe mental illness and addictions
  • Competence in relevant computer applications, particularly the Microsoft Office suite and Outlook
  • Two (2) or more years’ experience working directly with individuals with mental illness and addictions
  • Previous experience performing a case management function
  • Employment experience working with individuals with addictions or in a supportive capacity within the justice system
  • Current certifications in Non-Violent Crisis Intervention (NVCI), Standard CPR / First Aid, and/or ASIST
