Working within a multi-disciplinary team framework the Transitional Case Manager performs duties and responsibilities consistent with providing a psychosocial environment to assist with the rehabilitation and community integration of individuals involved in the criminal justice system, including those with serious and persistent mental health concerns, developmental disorders, acquired brain injury and/or concurrent disorders.

Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.
WHAT WE'RE OFFERING:
- What you'll be making - Starting rate for this role is $26.74 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($29.57 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($33.90 per hour for non-regulated, and $36.71 per hour for regulated)
- For internal applicants, grid level placement will occur as per the Collective Agreement
- When you'll work - 70 hours over two-week period; 8:30 am to 4:30 pm, Monday to Friday, with one unpaid hour for lunch. These hours may vary on occasion.
- What your benefits will be - After a 90-day waiting period, will be enrolled in extended healthcare benefits
- Your pension plan - Immediately enrolled in the Healthcare of Ontario Pension Plan (HOOPP)
HOW YOU WILL BE MAKING A DIFFERENCE:
Assessment -In collaboration with the individual:
- Complete intake/screener to support referral to Adult Therapeutic Court
- Gather health related documentation as required for Adult Therapeutic Court
- Have knowledge of specialized courts
- Have knowledge of community resources relevant to individuals, including other mental health and addiction services, entitlement and benefit programs, (e.g. ODSP, Ontario Works, etc.)
- Attend court sessions, as needed, and provide court support to individuals dealing with current or past charges
- Liaise with legal professionals and service providers to ensure a coordinated approach to service provision
- Advocate for client rights as necessary and support self-advocacy
- Identify, develop and maintain good links with a wide range of community resources
- Work collaboratively with other service providers to ensure a well-coordinated approach to provide support and/or discharge planning from detention centres/forensic hospital, etc.
- Assist the individual to identify personal goals and develop support plans that identify individual strengths and areas for personal growth and goals to meet determined needs.
- Provide 1:1 support services to a caseload of individuals with mental health concerns who are currently involved in the criminal justice system
- Provide skills teaching and support to clients to assist in goal achievement
- Provide assistance with financial management, vocational and occupational support, and discharge planning.
- Provide crisis support and crisis prevention planning
- Engage in group facilitation as required that supports clients in completing their established goals for court diversion
- Conduct assessments, treatment planning, and reporting for the Mental Health Diversion Program
- Provide information and referral support to individuals, family members, and service providers
- High level assessment skills to assess clients’ mental, physical and emotional functional abilities and needs.
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.
We require the following qualifications:
- University degree in a relevant area of study, preferably in the area of social work, psychology, or occupational therapy; a degree in nursing will also be considered equivalent when combined with educational or experiential specialization in psychiatric nursing (primary proof of credentials will be required during the hiring process i.e. original degree/diploma/certificate or official transcript, for ongoing education, an unofficial transcript will be accepted)
- Current registration in good standing with a relevant professional regulatory body
- Valid Driver’s Licence and access to a personal vehicle
- Vehicle insurance in compliance with CMHA TVAMHS policy
- Current, clear Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly
We prefer the following qualifications:
- Demonstrated knowledge and skill in clinical assessment, functional assessment, service planning, relapse prevention, and client advocacy
- Broad knowledge of, and practice in, evidence-based treatment approaches to working with individuals with severe mental illness and addictions
- Competence in relevant computer applications, particularly the Microsoft Office suite and Outlook
- Two (2) or more years’ experience working directly with individuals with mental illness and addictions
- Previous experience performing a case management function
- Employment experience working with individuals with addictions or in a supportive capacity within the justice system
- Current certifications in Non-Violent Crisis Intervention (NVCI), Standard CPR / First Aid, and/or ASIST
