The Housing Navigator provides client-centered, community-based services to individuals referred to the Community Homes for Opportunity (CHO) program, assisting them and their families in navigating the assessment, prioritization, and placement process within CHO. The position also serves as a liaison with referring partners, prospective tenants, and their families is part of the broader Supportive Housing intake / navigation Community of Practice.

WHAT WE'RE OFFERING:
- What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated); for internal applicants, grid level placement will occur as per the Collective Agreement
- When you'll work - 70 hours biweekly; Monday-Friday 9am-4:30pm or Compressed Work Week: Monday-Thursday 8am-4:45pm, one Friday off and the other 8am-12pm
- Your pension plan –Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:
- Review the CHO Intake Referral Package, and ensure information complete and documents are uploaded into the client data base, consents obtained, requests additional information.
- Interpret and uses standardized assessment tools (GAINSS, OCAN, LOCUS, VI-SPDAT, OT, Risk Ax etc) and their results to help the client identify their goals and preferences, and fit for the CHO Program.
- Supports with waitlist management for the CHO Program, Monitors CHO Program Movement, and collaborates with CHO Team Leads and managers about admissions and discharges
- Advocate, refer and connect clients with appropriate services, supports, and resources (formal and informal) based on their needs.
- Assess situations requiring specialized services (e.g. specialized assessment) and arrange appropriate linkage and support
- Coordinate and communicate the completed CHO intake package with the CHO Homeowner partners and follows up on any questions or concerns.
- Assist clients eligible for admission to the program to select a home placement from the portfolio of CHO homes to secure safe and suitable housing for their needs.
- In conjunction with Managers and Residential Service Liaisons, monitors the ongoing quality of profiles on the HomeChoice virtual tours platform and makes recommendations to enhance the quality of individual profiles as well as the platform as a whole, where appropriate.
- Provide needed information and education to key stakeholders about mental illness, addiction, coping skills, services, resources, self-help and consumer initiatives/supports
- Provide information on exclusion criteria and reasons for non-eligibility for the CHO program to referral sources
- Coordinates tours, intake meetings, and provides admission information with Program Support and Property Services to flow necessary information on new tenancies
Assessment and Goal Planning:
- Assess clients’ strengths, abilities, needs and preferences as well as social and environmental factors impacting the client, and any vulnerability and risk factors.
- Ensure screening and assessments are completed and reviewed according to policy.
Communication and Decision Making:
- Maintain open communication with client’s professional and natural supports in regards to client’s referral and recovery.
- Make decisions both independently and collaboratively.
- Complete required documentation related to clients as per organization standards.
- Complete other required documentation (e.g. monthly time sheets, vacations requests, travel claims, expense claims) within the required time frames.
- Provide input into decisions with team concerning operations of the program, policies of the organization and accreditation standards.
- Professionally liaise with community workers and agencies regarding the Supportive Housing Programs.
- Ensure effective communication with Director, Manager, staff team, and external treatment teams concerning each client’s progress.
- Identify, promote resolution, and management of all emergency situations concerning clients, staff, and property.
- Knowledge of issues pertaining to confidentiality, such as the shared Common Client Record (CCR) and consent to release personal information process to ensure client’s confidentiality is maintained at all times.
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
We require the following qualifications:
- Post-secondary degree or diploma in a program relevant to mental health or social services (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Current, satisfactory Police Vulnerable Sector Check (PVSC)
- Valid driver’s licence and access to a vehicle with adequate insurance to carry out job duties
- Demonstrated ability to attend work regularly
We prefer the following qualifications:
- First Aid & CPR, WHMIS Training
- Registration with a college or professional body associated with their discipline; employment experience in the mental health field
- Minimum of one-year work experience in the mental health and addictions field
- Experience working with people with housing instability
- Working knowledge of the Mental Health Act, and an awareness of trends in the provision of support services to adults with serious mental illnesses and addiction
- Health and safety knowledge
- Crisis intervention and prevention
- Knowledge of the Residential Tenancies Act
- Understanding of the Landlord Tenant Board mechanism
- Understanding and working knowledge of Landform Form N1-N13
- Knowledge of Psychosocial Rehabilitation
- Knowledge of mental health and addictions
- Knowledge of Assessments (OCAN, SPDAT, OT Ax, SW Ax, Risk Ax)
- Knowledge of internal support programs of CMHA TVAMHS
