Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 3 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
WHAT WE'RE OFFERING:
- What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)
- For internal applicants, grid level placement will occur as per the Collective Agreement
- When you'll work -
- Full-Time, Contract: 70 hours bi-weekly; Rotating days, afternoons & evenings, including weekends
- Full-Time, Permanent: 70 hours-bi-weekly; Rotating days, afternoons & evenings including weekends

HOW YOU WILL BE MAKING A DIFFERENCE:
- Ensure client’s Electronic record and paper file is complete and up-to-date
- Ensure goals are being worked toward and program expectations are being met
- Provide client updates at Team Meetings
- Create Team Approaches and/or Contract of Residency
- Follow through with primary responsibilities when a client’s Primary Worker is unavailable
- Assess client’s strengths, abilities, needs and preferences
- Build on client’s strengths, abilities and resources
- Assess social and environmental factors impacting the client
- Assist clients to develop and coordinate an Individual Rehabilitation Plan (IRP) based on their strengths, abilities, needs and preferences
- Review and revise IRP with client every four-six weeks, or more frequently if deemed necessary
- Motivate and support the client to work towards achievement of their IRP goals
- Include and encourage clients’ natural supports (e.g. family, significant friends) in the IRP, clarifying the roles each plays towards clients’ rehabilitation
- Regularly solicit feedback from the client and relevant community supports regarding participant’s mental health or addiction and progress towards his/her goals
- Maintain open communication with the client’s professional and natural supports in regards to client’s rehabilitation
- Completion of frequent Mental Health Status Exams within the first week of the client’s admission, and subsequently at a minimum interval of once per month
- Create or update client Crisis Plans
- Medication monitoring and education
- Ongoing liaison with RTF Nursing Staff Team to ensure a firm understanding of the clinical aspects of the RTF program
- Support clients with physical and mental health concerns, as well as addiction concerns, and liaison with medical staff
- Advocate for and support clients in accessing community resources
- Champion each client’s capacity for independent community living
- Facilitate and implement resident meetings, small groups, and activities
- Provide health teaching about mental illness and symptom management
- Assist clients to find and maintain suitable housing
- Crisis prevention and intervention
- Make decisions both independently and collaboratively
- Assist and coach clients in the learning and management/achievement of activities of daily living such as personal hygiene, daily chores, budgeting, socialization, etc.
- Support clients one-on-one with meal preparation
- Provide education and the experience of grocery shopping, and the organizational and budgeting elements related with this task
- Provide recreational community outings and activities to enhance quality of life for clients and further connect participants to their community
- Collection and accurate documentation of monies for rent and groceries
- Transition planning with the client and community support to facilitate a seamless transition to the community
- Completion of a Discharge Summary upon client's transition from RTF outlining progress made in the program and recommendations for the future
- Offer and provide follow up support to client in the community
- Complete required documentation related to clients as per organization standards
- Complete other required documentation (e.g. monthly time sheets, vacations requests, education requests, travel claims, expense claims) within the required time frames
- Minor maintenance of the facility such as repairs, common areas, grounds, resident rooms, etc.
- Make decisions with team concerning operations of the program, policies of the organization and accreditation standards
- Professionally liaise with community workers and agencies regarding the RTF programs, referrals, intakes, and waitlist
- Ensure effective communication with Director, Manager, staff team, and external treatment teams concerning each client’s progress
- Identify, work to prevent and promote resolution and management of all emergency situations concerning clients, staff and property
- Knowledge of issues pertaining to confidentiality, such as the shared Common Client Record (CCR) and consent to release personal information process to ensure client’s confidentiality is maintained at all times
WHAT YOU WILL NEED TO APPLY:

We require the following qualifications:
- University/college degree or diploma in the social science field (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Valid Driver's Licence and access to a vehicle with at least $2 million in liability insurance to transport clients
- Ability to attend work regularly
- Knowledge of Psychosocial Rehabilitation
- Experience working residence setting
- Health and safety knowledge; knowledge of safe food handling procedures
- Crisis intervention and prevention
- Knowledge of psychiatric medications
- Knowledge of community resources
- Applied Suicide Interventions Skills and Training (ASIST)
- First Aid & CPR
