The Addiction Case Management Clinician will provide a rapid response to individuals seen at the Woodstock General Hospital who can be appropriately referred for addition and mental health-related concerns.
This role is a direct service, clinical position that provides crisis intervention, brief transitional case management, system navigation, counselling and addiction treatment. In this position, you will collaborate with community-based addiction and mental health services to reduce avoidable, unnecessary, and repeat emergency department visits.

WHAT WE'RE OFFERING:
- What you'll be making - Starting rate for this role is $29.57 per hour (Level 1 of the Direct Service, Regulated Professional Grid);
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $36.71 per hour
- When you'll work - 70 hours biweekly; Monday to Friday, 8:30am-4:30pm

- Share in the provision of face to face Entry / First Contact / Intake Services as well as using virtual technology and clinical conferencing
- Provide intervention and short-term support to individuals experiencing an addiction or mental health crisis
- Facilitate early linkage with internal programs and/or other agencies and/or supports i.e. family, neighbours etc. by communicating and coordinating relevant information and providing case management services until linkages are created
- Provide screening for mental health and addiction issues and assessment of appropriateness of service
- Provide and facilitate referrals to addiction resources as appropriate
- Provide referrals to all appropriate social and health related services based on participant wishes
- Offer harm reduction strategies as well as abstinence strategies based on participants intentions and stated needs
- Provide discharge planning and support
- Deliver appropriate level of navigational support based on client capacity
- Share information about health and community/social services and link participants to same
- Provide assessment, treatment planning, referral, advocacy, preliminary case management, community treatment and education as appropriate to persons who have concerns about their own substance use or someone else’s.
- Facilitate community treatment groups as needed and appropriate
- Manage and meet targets for a clinical caseload and maintain direct service requirements
- Manage indirect services and administrative duties
- Engage in community development, community awareness, education and outreach as designated and appropriate
Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.
We require the following qualifications:
- Post-secondary education in a related field, e.g. BSW, MSW, post-graduate diploma in addictions or equivalent required (Primary proof of credentials will be required during the hiring process, i.e. original degree/diploma/certificate or official transcript)
- Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (as an RSW), the College of Nurses of Ontario, the College of Registered Psychotherapists of Ontario, or the College of Occupational Therapists of Ontario
- Current, clear Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly
- Crisis or suicide intervention training
- Experience with system navigation, coordinated care planning and discharge planning using the Transitional Discharge Model
- Demonstrated experience working from a Recovery Focused practice
- Experience in the treatment of addictions
- Experience and detailed knowledge of serious mental illnesses
- Knowledge of services in Oxford County
